Might be instructive to ask...that did not rack out or did not rack in using the hand crank it was intended to be used with. Might ask the same line of questions with automated remote racking assemblies, either manufacturer-built or third party. Answers are probably pretty scary because I'd say it has happened to me in different plants and different manufacturers about one out of every 10 times.

Yes I have in the past, but we do not allow our technicians to rack in circuit breakers energized anymore. We consider this energized work at our facilities because we are changing the state of the system when racking in breakers. We had one blow up several years ago, the paddle showed it was opened, but it actually was closed and our tech got hurt when he racked it in. We figured that our process wasn't that critical that we couldn't open the main, rack the breaker in, and then close the main back. We even have remote operators for our mains now, we don't want our techs near the equipment if we can help it.
