Have you ever used information from any of the informative annexes found in NFPA 70? Yes No We use a different standard Doesn't apply If so, what information/annex(s) did you use? If you use a different standard such as CSA Z462 etc., which one do you use? |

Our PPE selection was based on the 12 and 40 cal/cm^2 method. |
