Two of us at our firm seem to remember a requirement for the author's name and/or firm name to be required on AF labels. Were we dreaming? I can't find the requirement now! Thanks, Steve

NFPA 70E 130.5(H) 1 - 3 require the voltage, arc flash boundary and method to select PPE such as PPE category or incident energy and working distance but not both, site specific level, arc rating.

NFPA 70E does not require anything else to be listed on the label although people do add additional information such as study consultant name, date, LAB and RAB but none of this is mandated by NFPA 70E.

The creation date on the label can be very helpful, especially when doing periodic reviews (that check where you make sure the labels are current and in decent shape).

I also like to add the name of the person or company responsible for the label creation and, if possible, a reference project number. Knowing that this is not nearly as important as the required information, I keep these to the very bottom of the label and in a reduced font size.

I do not add company logos. I want as much room as possible on the label for the required information. I've seen labels where every level of contractor added their logo to the arc flash label (five of them in one case) and there was almost no room for the required info. I don't mean to poke any particular company, but the template labels in some of the major arc flash software packages are horrible for taking up major chunks of label space with their company logo. I've seen folks use these default templates without any changes.

Jim has an excellent video on brainfiller.com discussing what is required to be on the label as well as a strategy to simplify and somewhat future-proof the label so you don't necessarily have to change the labels every 5 years. Just verify they are still correct. You might check it out if you haven't already.

I'm a fan of Jim's advice on label simplification. I've had a few conversations with him about that. It works really well in situations where the electrical systems are being frequently modified.
