So, 2 days into a 5 day shutdown, I have to suspend a guy for 10 days. We were in the process of changing the controls from 480V to 120V in some MCC lines, when one of my guys failed to wear his PPE, installed a starter bucket, pinched a wire on the back side of the bucket, and shorted to ground. Luckily, he was okay, but he really impacted the job by putting me a man short. As well as costing himself a couple grand.
