Preliminary report at this link: http://www.msha.gov/FATALS/2010/2010MNM16prelim.pdf Preliminary report says 3 workers were installing ground fault indicator lights on 800 Amp breaker. Arc flash occurred, 3 injured, 1 died 2 days later. MSHA follows with more details in a "Fatalgram" usually in 2 or 3 weeks. The final report will be issued after a much longer time. |

Fatalgram now on MSHA site. The "Fatalgram" for the accident is now on the MSHA site. http://www.msha.gov/fatals/2010/FAB10m16.asp Remark, the Fatalgram indicates the breaker was bottom fed. Never my first choice for breaker feed. Also notice there is an opportunity to submit suggestions. |

After a quick glimpse of the report, it looks as though they assumed the Utility Supply was on the top of the breaker. Using a meter to check for voltages prior to starting their job would have saved his life... NEVER assume anything!! People forget that you can't see electrical, like you can with steam, water, etc... It WILL bite you if you are complacent and cut corners... |

The report clearly indicates the importance of testing for voltage using "Live-Dead-Live" procedure. Always verify line and load side terminals. |
