Standing Electrical Work Permit Template?

Does anyone know how I can get a template or view one based on routine electrical work.

For now I have been using a standard Engergized Electrical Work Permit and modified to say standing. I want to use this for documentation on opening <75kVA transformer fed 120-208 Panelboards for troubleshooting. Also, this would would document the worker has training and gone over the hazards to several different routine work situations.

I have seen a few .gov Standing Work Permits but contains more then electrical work.

Annual Work Permit for Electrical Work

Most of the ones I have seen are the same as what you use but we recommend you call them Annual Work Permits now to meet all the requirements of NFPA 70E's Energized Work Permit. This is difficult to do in a "standing" or even an annual manner but who can do what work without an energized permit can be established with proper training and adequate documentation.

Be certain you are documenting the safety training, task training, and Annual Supervisory Work Practice Observations required by NFPA 70E. These can also be helpful in the event of an audit or citation.

An annual permit should document all the workers by name or job classification (if you have task specific training in each job classification). This should get a regular review to meet compliance requirements thus why we recommend Annual. I got this idea from Terry Becker at ESPS.
